Use a public key. Add your website to the key's allowed origins first; test keys may also use http://localhost.

What it does for you
- Shows name, phone and email, then your workspace's lead fields, with clear labels and inline errors.
- Adds the captcha, a hidden spam trap and your consent text with a required checkbox.
- Sends each submission once (a new Idempotency-Key per submit) and shows a thank-you message.
- Sets no cookies and stores nothing in the visitor's browser.
